To Submit a Nomination, please include the following info (Please keep to THREE pages maximum)

  1. A clear description of the service, publication, project, event, or exhibit to be recognized.
  2. Indicate how you became aware of the contribution, publication, project, event, or exhibit.
  3. For individuals: address the scope of the personal accomplishments and the services rendered.
  4. For organizations: clearly describe the qualities and attributes which render the project, event, or exhibit an outstanding accomplishment and indicate the number of people who participated and the number of people served.
